When a young person turns 18

Young people with a continuing care package may also qualify for NHS continuing healthcare for adults. 

A new assessment is needed and adult care may differ. 

The NHS Buckinghamshire, Oxfordshire and Berkshire West Integrated Care Board (ICB) will support your young person during this transition. 

What to expect

A young person will usually:

  • have an initial checklist assessment at 16 years old, completed by a health or social care professional
  • have a full assessment after turning 17, if eligible after the checklist assessment
  • start an adult NHS continuing healthcare package when they turn 18
  • have a review around 3 months after turning 18 to check the care package is still right for them

If they are not eligible

If a young person is not eligible for NHS continuing healthcare for adults, they can have an adult social care needs assessment.
