What happens after referral
The NHS Buckinghamshire, Oxfordshire and Berkshire West Integrated Care Board (ICB) will follow the steps in the children and young people's continuing care national framework.
1. Pre-assessment checklist
A health or social care professional will complete a checklist to see if a full assessment is needed.
Parents cannot complete the checklist, but your views should be included.
2. Assessment
If a full assessment is needed, the ICB will appoint a health practitioner to carry it out. They will use a decision support tool.
3. Decision
A panel will review the assessment and decide if your child has continuing care needs.
You should receive the decision verbally and in writing within 6 weeks.
4. Arranging support
If eligible, the practitioner will work with you and other professionals to plan the care package.
You can ask for a personal health budget to arrange the agreed care.

5. Review
The ICB will review the care package after 3 months, then every year.
If your child is not eligible
If the panel decides your child does not have continuing care needs, they will:
- tell you verbally and provide a clear written explanation
- explain your right to ask for a review
