How to apply

You need a referral from a health or social care professional.

Professionals who can refer include:

• GPs
• hospital practitioners
• community and specialist health services
• community nursing teams
• school nurses
• Child and Adolescent Mental Health Services (CAMHS) professionals
• social workers  

Referrals are made to the NHS Buckinghamshire, Oxfordshire and Berkshire West Integrated Care Board (ICB).
