Adult Social Work Team

Social care workers work with you to find solutions to your problems, they can provide support, resources, and advocacy.

Depending on your situation and your needs, you or your carer will be allocated a social care worker who has the appropriate skills and knowledge. They will also help you to consider your strengths, things you can still do independently, and where your family, carers, or wider community can help. During this conversation, they will establish whether you have any eligible needs and what support you require.

Mental health support through social care

Our mental health social care service supports adults aged 18+ with complex or serious mental health needs. We also offer a dedicated service for older people (75+) and younger individuals with significant frailty or memory issues. Learn more on the support with mental health through social care page.
