Preparing for Adulthood Team

The Preparing for Adulthood Team works with young people aged 14 to 25 to help them transition into Adult Social Care.

Services are free.

How to access the service

Schools, parents and healthcare workers can make referrals to the Preparing for Adulthood Team.

To be eligible for support, you must meet at least 2 of the support needs from the Care Act 2014.

Assessing your needs

The team will usually do a needs assessment when they receive referrals. This is called a Self-Directed Assessment.

The team will focus on helping young people be as independent as possible. They will:

  • look at transport needs for getting to and from training, work or school and offer travel training options where appropriate
  • help find groups or activities
  • check health needs and what other services
  • work with parents, school or workplace to ensure a smooth transition from one school or placement to the next
  • look at technology-based support aids to promote independence - if this is required
  • make a referral to our supported employment service for help finding training or volunteering
  • offer advice, support and guidance for the young person’s future choices such as looking at housing options
