Hospital Liaison Team
If you are in hospital, the hospital will contact adult social care on your behalf and will offer support to facilitate a safe and timely discharge.
The hospital discharge team will undertake an assessment and put in any services that may be necessary such as a reablement service. Should you require ongoing support, they will talk to you about what is available.
