Support care leavers with their health and wellbeing

We want all children and young people to be healthy, and have the help you need when you want or need it.

Support with health and wellbeing

Your Personal Advisor (PA) can provide you with 1:1 support around health and wellbeing, and link you with other services to support you with health needs identified.

Your PA can support you with:

  • Healthy living and healthy eating advice
  • Registering with a GP, dentist, or optician
  • Accessing mental health support and counselling services
  • Moving on from Child and Adolescent Mental Health Services (CAMHS) to equivalent adult services
  • Information about local health drop-in centres
  • Accessing sexual health advice, guidance, contraception, and sexual health clinics
  • LGBTQIA+ matters, and will support you to understand where you are, and help you access any support groups or organisations in your area.
  • Referral to local drug and alcohol services.
  • Free prescriptions: You might be entitled to a pre-payment certificate for prescriptions. This certificate means you won’t need to pay for items prescribed by your doctor (GP) for between 3-12 months. Your PA can help you check if you are entitled to this, and help you apply for the certificate.
  • Stop smoking: We can help you get free stop smoking aids like patches and gum, and you can speak to an advisor via SmokeFreeLife Berkshire.

Heath Appointments

If you would like support to attend a medical appointment or find a new doctor and dentist when you move, your PA can help you with this.

Under 18s will be supported to:

  • Access regular doctor, dentist and optician appointments
  • Purchase glasses

Health Summary

All care leavers will receive their Health Summary when they turn 17 or in the year leading up to their 18th birthday. This is a small booklet which contains your:

  • Health history, such as details about your birth
  • Present medical conditions
  • Immunisation history
  • Other useful health information

If you come in to care after your 17th birthday your initial health assessment will provide a more detailed outline of your health needs.

You will be offered a last health review with a nurse before you are 18, where you can discuss your Health Summary.
