1. Do you use online forms for residents to complete using a web browser?
Yes.

For all questions, we would like the responses that relate to the latest figures that you have available (please state the date period in your response) and, ideally, the preceding 12 months.

1. Please advise what the postholders are who personally support the CEO in his/her work eg PA, executive support officer, business manager, admin support or similar.
Our Chief Executive and our directors are supported by Executive Personal Assistants.

1. How many individuals is the local authority acting as an appointee for, please?
14 appointee cases.

2. How many of these individuals (in area or out of area) pays adult social care charges to the council for non-residential services?
Four.

Please can you fill out the below questions relating to the social care case management system and joint funding agreements used at your organisation for both Adult and Children's Social Care if relevant.

Note for all of the following questions – please provide your metrics / responses pre-Covid levels (i.e. normal operation of air-conditioning).

1. Is your system an air-conditioning system or recirculated air system?
Mainly fresh air intake and 2 x air-mixing plenums.

1. When a Looked After Child (in the care of your local authority) moves placement:
a) Are sturdy suitcases or luggage always offered to assist with transporting their belongings?
Yes.

1. Please can you send us copies of all documents currently used by your housing officers to conduct these housing affordability assessments (such as guidance notes, briefings, training sessions and template forms).
