SEND support for young people leaving care
Support for young people and young adults with special educational needs and disabilities
Need a bit of support?
You’re not on your own. If you are 25 or under and have special educational needs or disabilities (SEND), there’s help available for you.
The SEND Local Offer is a simple website where you can find out:
- What support you can get
- How to get that support when you need it
What can the Local Offer help with?
You can use it to:
- Understand what to do if you need extra support
- Get advice about becoming an adult in a way that works for you
- Learn about getting ready for work, training, or your next steps
- Find activities, clubs, and things to get involved in
- Learn about your housing options
There is a section called Preparing for Adulthood, which is helpful if you are thinking about:
- Work and future goals
- Becoming more independent
- Planning what comes next in your life

A SEND Local Offer Co‑ordinator can help you find what you need.

Wokingham SENDIASS
Wokingham Special Educational Needs and Disability Information, Advice and Support Service (SENDIASS) is a service that can help you.
SENDIASS is free, confidential, and independent service that supports young people with Special Educational Needs and Disabilities (SEND) up to age 25, as well as parents and carers.
They can help you understand support around:
- Health
- Social care
- Personal budgets
- Education
They explain things clearly and help you understand your choices so you can feel more confident.
You don’t have to figure everything out on your own, there are people ready to help you every step of the way.

Your Personal Advisor (PA)
Your Personal Advisor (PA) is someone who is there for you.
They will:
- Listen to you
- Support you
- Help you think about your next steps
Your PA can help you with:
- Sharing your views and being part of planning your future
- Making choices that feel right for you
- Building confidence and becoming more independent
- Learning everyday life skills
- Moving from children’s services to adult services when the time is right for you
Your PA is there to help you feel ready, confident, and supported.
