Work with your Personal Adviser to explore your goals, needs, and next steps

When you turn 16, your Care Plan becomes a Pathway Plan. It’s all about you—where you’re at right now, what matters to you, and how we can support you to get ready for independence. It also looks at your goals and what you want to do next.

This is your space to talk about things like: 

  • What’s on your mind and what matters to you 

  • What kind of support you need at the moment 

  • What you’re hoping for in the future (even if you’re still figuring it out)  

  • How we can back you up and help you along the way 

Your Pathway Plan belongs to you. You will work on it with your Personal Advisor (PA), who will support you and make sure your views are heard. 

We’ll catch up with you regularly to see how things are going and make any changes if they’re needed. We’ll have a proper review at least twice a year, and more often if something big changes in your life.

Care Plan
Your care plan is written by your social worker and has information about you, your placements, and what your plans might be for the future. It is important that you take part in writing your plan. It should be updated regularly as things change.

Pathway Plan
A pathway plan is your plan and is a written document that looks at your plans and goals and what is needed to help you achieve them. It is important that you take part in writing your plan. This helps identify your current skills and any new skills that you need to develop to help you move towards being more independent.

Review
Before turning 18, you will have an Independent Reviewing Officer (IRO), who will lead your reviews and make sure your views, wishes, and feelings are listened to. They will also check that your care plan is meeting your needs and that everyone involved is doing what they have agreed to do to support you. They ensure that your plan is reviewed every 6 months. 

When you turn 18 your personal advisor will make sure your pathway plan is reviewed every 6 months or when there is a big change in your life.
