Your housing with care and support decision

Before we make a decision, you will have had:

  • an Adult Social Care Needs Assessment
  • a Financial Assessment

These assessments help us decide:

  • if you can get adult social care support
  • if you can get housing with care and support

You will get a copy of your assessment.

If you are not eligible

If you cannot get adult social care support, we will still help you.

We will:

  • give you advice and information
  • tell you about other support, such as: 
    • charities
    • community groups and services

If you are eligible for housing with care and support

A social care worker will:

  • talk with you about the support you need
  • agree with you what services can help
  • work with you to create a care and support plan

Your care and support plan

Your plan will show:

  • what you can do on your own
  • what you need help with

It will also include:

  • any money you will receive
  • any services we will provide
  • any equipment we will give you

  • We will talk about the type of home and support you need.
  • We will work with other services to help find a home that is right for you.
  • This may take some time, but you’ll be kept updated.
  • Options are explored with you, focusing on what matters to you. They must meet your needs and be available.

Finding and moving into the right home

  • When a suitable option is found, it’s discussed with you and, if needed, your family or advocate.
  • You’ll have the chance to visit the home, meet the people there, and see if it feels right.
  • Next steps are agreed for moving in and making sure your support is ready.
