Your housing with care and support decision
Before we make a decision, you will have had:
- an Adult Social Care Needs Assessment
- a Financial Assessment
These assessments help us decide:
- if you can get adult social care support
- if you can get housing with care and support
You will get a copy of your assessment.
If you are not eligible
If you cannot get adult social care support, we will still help you.
We will:
- give you advice and information
- tell you about other support, such as:
- charities
- community groups and services
If you are eligible for housing with care and support
A social care worker will:
- talk with you about the support you need
- agree with you what services can help
- work with you to create a care and support plan
Your care and support plan
Your plan will show:
- what you can do on your own
- what you need help with
It will also include:
- any money you will receive
- any services we will provide
- any equipment we will give you
What you need to do
You will need to:
- Create a Housing Online account
You can get help by:
- reading our easy read guide on how to register for a Housing Online account.
After you register, you will need to:
- complete the New Housing Application Form

What happens next
- We will talk about the type of home and support you need.
- We will work with other services to help find a home that is right for you.
- This may take some time, but you’ll be kept updated.
- Options are explored with you, focusing on what matters to you. They must meet your needs and be available.
Finding and moving into the right home
- When a suitable option is found, it’s discussed with you and, if needed, your family or advocate.
- You’ll have the chance to visit the home, meet the people there, and see if it feels right.
- Next steps are agreed for moving in and making sure your support is ready.
