Finding the right housing and support for you
How we help you find the right support
If you want to live in housing with care and support, we will first carry out an adult social care needs assessment with you to see if you're eligible.

Who can have an assessment
If you are 18 or older, you have the right to ask for a social care needs assessment.
This right is part of the Care Act (2014).
You can read the UK Government's easy read booklet to the Care Act for more information.
About the social care needs assessment
A social care needs assessment is a meeting where we talk with you about your life and what support you might need.
We will ask you questions about:
- how you are feeling
- how you manage day-to-day activities
This helps us understand your situation and the kind of support that could help you.
We will also talk about services that can help you to:
- stay independent
- need less support
- delay needing long-term care
We will talk about these services during your assessment, even if you don’t qualify for full support.
Checking your financial assessment
We will also look at your finances in a financial assessment.
This helps us understand whether you may need to contribute towards the cost of your care.
What we use the assessment for
The assessment will help us decide whether you are eligible for support from us and what support you may receive.

If you're not eligible for support
If you can’t get support from us, we will still:
- give you advice and information
- tell you about other groups that might help, like charities and community services
