To live in housing with care and support, we will first do an adult social care needs assessment with you.

A social care needs assessment means we will meet with you and talk about your life.

We will ask you questions about:

• how you are feeling
• how you manage day-to-day things

This helps us understand what support you might need.

We will also check your money in a financial assessment.
