Support with mental health through social care

Our mental health social care service supports adults aged 18 and over who have complex or serious mental health needs.

We also have a dedicated Older People’s Mental Health Service for people aged over 75, or younger people with significant frailty or memory problems.

Our team includes mental health social workers, social care practitioners, and occupational therapists. They offer support to people who:

  • have an on-going mental illness
  • may have had an admission to hospital and may be eligible for S117 aftercare
  • require ongoing care and support

We can help by assessing needs and developing support plans to:

  • developing fulfilling daily routines
  • ensure people are supported by various community-based services regarding social isolation, housing and benefits
  • prevent admission to hospital
  • help people to stay safe and manage risks
  • support carers and families

We work closely with health professionals across Berkshire Mental Health Services to make sure the support received is joined up and tailored to needs.

How to Access Support

  • Already receiving support from a Wokingham mental health team?
    Speak to your allocated worker and ask for a referral for a social care assessment.
