NHS health checks

Free community health checks

Our Community Wellness Outreach Team offers free health checks to help identify potential health issues early and support you to stay well. 

These checks are designed to make preventative healthcare more accessible in the community. While some people may be eligible for an NHS Health Check through their GP, not all practices are able to offer this service. Our community health checks provide an alternative way to access this important support. 

Health checks are quick, friendly, and focused on helping you understand your health and next steps. 

Eligibility

You may be eligible if either of the following applies: 

  • You are aged 30 to 74 years of age 
  • Live in and/or are registered with a GP in Wokingham 
  • You are not currently on medication or receiving treatment for: 
    • High blood pressure 
    • Heart failure 
    • Diabetes 
    • High cholesterol 
    • Chronic kidney disease 
  • You have not had a health check within 5 years  
  • Or you are eligible for an NHS Health Check

When you’re not eligible

You are not eligible for this service if you have already been diagnosed with one of the following conditions, as you will already be receiving care from your GP: 

  • Heart disease 
  • Atrial fibrillation 
  • Angina 
  • Heart attack 
  • Stroke (including Transient Ischaemic Attack or TIA) 
  • Diabetes 
  • Chronic kidney disease (CKD stages 3–5) 
  • Hypertension (persistent high blood pressure) 
  • If you are being treated with statins for high cholesterol 
  • If it hasn’t been 5 years since your last health check  
  • If you do not live in or are registered with a GP in Wokingham 
  • If you are not within the ages 30 to 74

Whether you are entitled to an NHS Health Check or not, you can see how you, and your heart, are shaping up for your age by using the check your heart age tool on the NHS website.
