Our Lady of Peace Catholic Club Limited Club Premises Certificate 2005 and Premises Licence 2021

1. Would you please provide:
a) A copy of the Embedded Restrictions carried over from the Licensing Act 1964 which were attached to the Our Lady of Peace Catholic Club Ltd Premises Certificate dated 24/11/05
Please see attached, however, we are not sure if this was the exact document that was sent with the 2005 Licence as our records do not go back to include this.

b) A copy of the Club’s Premises Licence issued in 2021

c) A statement confirming or denying that the Club Premises Certificate 2005 and its conditions still apply to the Club?
We have not had a formal surrender of this as far as we can see.
