1. The total number of current EOTAS packages.

2. The lowest, highest and median annualised cost of current EOTAS packages. If possible, please also provide the mean/average annualised cost.

3. Of the current EOTAS packages, the number which include each of the following forms of provision:
o Individual/1:1 tuition
o Alternative provision
o Speech and Language Therapy (SALT)
o Occupational Therapy (OT)
o Sensory integration or sensory-regulation provision
o Swimming or hydrotherapy
o Gymnastics, climbing or other structured physical/sensory-motor provision
o Equine therapy, therapeutic horse riding or other equine-based provision
o Community-based educational provision/activities
o Life-skills or independence provision
o Educational trips or admission costs for educational/community activities
o Specialist educational resources/equipment
o Sensory equipment
o Travel/transport funding associated with accessing EOTAS provision
o Personal Budgets
o Direct Payments.

4. For EOTAS packages containing individual tuition, please provide, if held:
o the lowest, highest and median number of tuition hours per week currently funded; and
o the lowest, highest and median hourly rate paid for individual tuition.

5. For packages containing alternative provision, please provide, if held, the lowest, highest and median number of days/hours per week funded.

6. How many current EOTAS packages contain provision delivered:
o solely at home;
o through a combination of home and community/alternative provision; and
o predominantly within community or alternative provision.

7. How many current EOTAS packages are:
o entirely commissioned and paid for directly by the Local Authority;
o entirely managed through a Personal Budget/direct payment; and
o funded through a combination of LA-commissioned provision and Personal Budget/direct payments.

8. Where a Personal Budget/direct payment is used for EOTAS, please provide the lowest, highest, median and average annual value of those budgets/payments, if this information is held in a readily retrievable form.

9. Please confirm whether Wokingham Borough Council currently has any maximum financial limit, indicative budget, funding band or standard expected cost for an individual EOTAS package. If so, please provide the relevant figure(s) and policy/document in which this is set out.

10. Please confirm whether there are any categories of EOTAS provision which Wokingham Borough Council will not ordinarily fund through a Personal Education Budget/direct payment, and provide any policy or guidance relied upon when making those decisions.
