Please provide the following information held by the council relating to statutory Education, Health and Care (EHC) Needs Assessments from 2014 onwards, broken down by academic year or calendar year (whichever format is routinely held).

Outcomes for disadvantaged children
For children attending state-funded schools who:
• Were eligible for Free School Meals (FSM) at the time of assessment; and/or
• Were from minority ethnic backgrounds; and/or
• Were from low-income households (where such data is held),
please provide, for each year:
1. The number of requests for an EHC Needs Assessment received from these families.
2. The number of EHC Needs Assessments approved based on evidence provided.
3. The number of EHC Needs Assessments rejected or refused. Conditions for refusal.
4. The number of assessments completed that resulted in an EHCP being issued.
5. The number of assessments completed that did not result in an EHCP being issued.

Overall assessment numbers
For each year from 2014 onwards, please provide:
6. The total number of EHC Needs Assessments conducted.
7. The total number that resulted in an EHCP being issued.
8. The total number that did not result in an EHCP being issued.

School type and socioeconomic breakdown
please provide for each year:
9. The number of children issued with an EHCP who attended:
o State-funded schools;
o Independent/private schools;
o Specialist schools.
10. Any data held regarding:
o FSM eligibility; and
o Other indicators of socioeconomic disadvantage,
for children who were issued with an EHCP.
