Please provide the following recorded information relating to dementia support services:
1. Whether the Local Authority commissions, funds or provides CST (Yes/No).
a) Organisation(s) delivering CST.
b) Number receiving CST in the most recent 12-month period.
c) Whether other structured cognitive support programmes are funded and their names.
d) Waiting list status, waiting-list size and average waiting time.
e) Delivery settings and referral sources.
f) Primary delivery model (LA/NHS/voluntary/independent/mixed).

a) Number of people with dementia receiving Adult Social Care services.
b) Number of dementia-specific day service places and whether provision has increased, decreased or remained stable over three years.
3. Dementia carer support and training provision.
4. Falls prevention pathway linked to dementia support (Yes/No).
5. Digital cognitive interventions commissioned, piloted or evaluated and provider names.
6. Staff groups delivering CST and whether workforce capacity is a barrier.
