For the purposes of this request, “specialist supported housing” refers to supported living or other supported accommodation arranged or commissioned by the local authority for adults with eligible care and support needs. If your authority uses a different internal definition, please apply your closest equivalent, and note this in your response.
Please provide the information as a snapshot as of February 1st 2026, or the nearest available date, if the preferred date is not possible.

Please provide aggregate counts only. No personal or identifiable information is requested.

1. The number of adult social care referrals (including open cases referred internally within adult social care for placement or accommodation sourcing) that are currently awaiting a specialist supported housing outcome because the absence of suitable accommodation is preventing placement or move-on.
NOTE 1: This should include, where applicable: referrals awaiting supported living or other supported accommodation, referrals held by placements, brokerage, or commissioning teams, and referrals where housing availability is the primary barrier to placement or discharge.
NOTE 1a: If you do not record this exact measure, please provide the closest equivalent dataset you do hold, for example, referrals awaiting placement, supported housing waiting list, brokerage referrals, or move-on referrals where housing is the blocker.

2. Where recorded for the referrals identified in Question 1, please provide a breakdown by primary need category, using your authority’s statutory or internal classifications, for example, Learning Disability / Autism, Mental Health, Physical Disability, Older People.
NOTE 2: If Learning Disability and Autism are recorded as a combined category, please provide the combined figure.

3. Where recorded for the referrals identified in Question 1, please provide a breakdown by geography, for example neighbourhood, locality, patch, or equivalent.
NOTE 3: If district-level data is not held, please provide the breakdown by the nearest geographic proxy you do hold and specify the geography used.

4. Please provide a list of all providers currently appointed to the Council’s supported living frameworks and/or dynamic purchasing systems, including the name of each provider, the supported living lot(s) or service categories they are appointed to, and the start and end dates of the relevant framework agreement(s) or DPS. If this information is already publicly available, please provide a link to the relevant document or webpage.

5. Please provide the name and job title of the most senior person who is responsible for determining adult social care referrals.

6. Please provide the direct email address and direct phone number of the person/s named in answer to question 5.
