1. The names and job titles of all individuals who are part of your social work management team, specifically those working in:
• Children’s Services
• Adult Social Care
• Safeguarding Adults
• Hospital Discharge
• Adults Access / Front Door / First Response
• Mental Health
• Adults Community / Neighbourhood / Locality
• Any equivalent senior or team management roles within statutory social work functions.
The professional contact details for each of the above individuals, including:
• Work email address
• Telephone number (direct or team contact)
• Office location (if available)
If it is helpful, I am seeking to understand the current leadership and team management structure within your social work services.
