How to get a short break

Refer yourself (self-referral) for short breaks

If you think you are eligible for short breaks, download and fill in the self-referral form.

Return your completed form to our duty triage team by emailing triage@wokingham.gov.uk or by post to:

What happens next

After we receive your form, we will:

• create a record for your child on our system, if one does not already exist
• pass your form to the Short Breaks and Early Help Team
• contact you within 5 working days of receiving your form

Short breaks direct payments

A short break direct payment is money you can receive from us to buy short break services or support on behalf of your child, rather than having us arrange them for you.
