Passports to explain your health needs

E-Health passport

An e-Health passport is part of your digital care record. It helps healthcare staff understand what is important to you and how they can support you. 

You can share information about:

  • how you prefer people to communicate with you
  • any sensory, access or neurodiversity needs you have
  • your faith, culture, gender identity or pronouns
  • any support you need with appointments or forms

You do not need to fill in a form. Ask a member of healthcare staff to update your e-Health Passport.

Neuro-inclusion passport

A Neuro-inclusion passport can also help you tell health staff about your needs.

You can use it to share information about:

  • what helps you feel comfortable and supported
  • things that may make you feel anxious or overwhelmed

You can fill it in yourself or ask someone to help you.

Bring your completed passport to your appointment. Your clinician will add the information to your e-Health Passport so that staff understand your needs and can support you in the best way.

Hospital passport

If you have a learning disability, a hospital passport helps hospital staff understand your needs and how to support you.

You can share information about:

  • what is important to you
  • things you like and do not like
  • information that helps you feel comfortable and safe

If you go to hospital:

  • take your hospital passport with you
  • ask staff to add a copy to your medical notes
  • keep it by your bed if you stay overnight so staff involved in your care can read it and support you in the best way
