How to plan for a transition

How you can help your child prepare well for a transition.

Talk to your child's school SENCO

If you need someone to talk to about your child's transition, speak to your child's special educational needs co-ordinator (SENCO).

Visits to the new education setting

Visiting your child's new school may help prepare your child for transition. Talk to the SENCO at your child's current school or nursery to discuss and arrange a visit.

Sharing information

Your child or young person's current school will pass on information that will help the new school or college support them. These include:

  • educational records such as individual education plans
  • Education, Health, and Care Plan (EHCP)
  • review paperwork
  • intervention records
  • diagnostic assessments
  • professional reports
