Partnership with Health Team

The Partnership with Health Team - PWHT - is a social services team within Wokingham Borough Council.
 
We provide services to Wokingham borough residents over the age of 18 who have been admitted to hospital, and may need advice and/or community care services.
 
How and why would I be referred to the Partnership with Health Team?
 
Some patients may need support to enable them to be safely discharged from hospital. They can request help - make a referral - from the Partnership with Health Team.
 
Referrals to the team can be made by anyone, including patients, relatives and hospital staff. Referrals can be made by telephone, in person or by fax/email. 
 
What happens after I have been referred to the Team?
 
Once the team has received your referral, your individual case will be prioritised according to the level of urgency. A care manager will then usually visit you in hospital in order to assess your individual needs.
 
What services may be provided?
 
Following an assessment of your needs, various services may be offered to you to enable you to be discharged from hospital. A care manager will explain and discuss the options with you. Services that can be provided include:
 
  • Referral to our short term reablement team - START
  • Home care services - support at home with personal care and other tasks of daily living, such as domestic tasks.
  • Meals delivered to your home
  • Day care
  • Residential or nursing home care
  • Advice on benefits
  • Information about household services
 
Will I have to pay?
 
There is no charge for an assessment of your needs. 
 
A charge may be made for any services provided, dependent upon your individual circumstances. You will be asked to complete a financial assessment form. 
 
You can choose to pay the full cost of the service. Under ‘Fairer Charging’ you will be offered a visit by the Benefits Liaison Team.  This will include a benefits check. Your care manager will explain this to you.
 
Services for carers
 
A carer is someone who regularly provides a substantial amount of care. Carers are entitled to an assessment of need in their own right, and may be eligible for services to support them in their caring role.
 
What happens following my discharge from hospital?
 
Following your assessment of need, a care manager will discuss and agree your care plan with you. You will be given a copy of your care plan to keep.
 
You will have the opportunity to comment in writing on the content of the care plan.
 
During the first weeks following your discharge from hospital, a care manager will monitor any services that we have provided for you. 
 
Normally, after 2-4 weeks a social worker will organise a review meeting with you in order to ensure that your needs are being met. 
 
What happens next?
 
The PWHT aims to provide a service for 4-6 weeks following your discharge from hospital. After the review meeting, if you continue to receive services your case will be transferred to our teams in the community who will be responsible for monitoring and reviewing your care plan.
 
At your review meeting, it may be identified that additional support or services are needed and in these circumstances your case would be transferred to one of our community teams for any ongoing care management support that has been identified. 
 
Due to the increasing demands on social services there may be a delay in how quickly the community teams can provide any such support.
