The Wokingham Community Team for People with Learning Disabilities - CTPLD - is a team of social care and health professionals who work with adults, 18+, with learning disabilities and their families.

How do I contact the team?
 
You can contact the team through letter, fax or email or by visiting the office and speaking to a duty worker. The duty worker is available between 9am and 5pm Monday to Friday.
 
What happens next?
 
You will be asked for your details and given any immediate advice. Dependent on your needs you might be referred to a care manager for a community care assessment or directly to one of the health professionals in the team. A letter will be sent to you explaining what is going to happen and then someone from the team will then contact you to make an appointment.
 
What is a community care assessment?
 
An assessment is a process that finds out about your circumstances and the help you might need. It looks at things like:
 
  • Where you live and if this needs to change
  • The support you need
  • Finding suitable day occupation or work
 
What if I am a carer?
 
Carers have a right to a carer’s assessment. This considers the impact that being a carer has on the carer’s health and well-being, employment, study, and leisure interests.
 
Can everyone get a service?
 
Unfortunately, the council are unable to provide community care services for everyone. Because we are unable to help everyone, the council agrees and publishes the situations where we can help people. This is called ‘eligibility criteria’. A leaflet is available from our offices to explain this.
 
What happens if I am not eligible for a service?
 
We will provide information and advice about other community opportunities that might be available like drop-in services, support groups etc.
 
Some people who are not eligible for a community care service may be eligible for a service from the health professionals in the CTPLD. We will discuss this with you as part of your assessment.
 
What can I get help with?
 
This could include:
 
  • Information, advice and support to get somewhere suitable to live
  • Support in your home with daily living tasks
  • Access to day opportunities and / or employment support
  • Short-breaks
