Transition to further education
From the age of 13 or 14 years-old, all young people are supported by their school to consider the options for further education and career choices.
Statement of Special Educational Need
For young people with a Statement of Special Educational Need (SEN), the transition process starts at their annual review in school Year 9. While they remain at school, the reviews continue every year until their 19th birthday. At these reviews, all areas of a young person's life will be looked at, including education, work, residential, supported and independent living.
Year 9 review
As well as family, carers and school staff, a number of professionals will be invited to the review in Year 9. This includes:
- A Connexions Service Personal Adviser
- Educational Psychologist
- Social Worker
- Health Professionals - if involved in a young person's care
- Educational Support Service - if involved in a young person's care
- SEN Officer
From this review, a Transition Plan will be drawn up, detailing future plans and how they will be met. This will continue to be referred to and adapted at all future reviews.
Without a Statement of Special Educational Need
Young people who don't have a Statement of Special Educational Need will receive support from the Connexions Service and their school during from the time they are in school Year 9.
Support service for people with disabilities
Connexions is the Government support service for all young people aged from 13 to 19 years - up to 25 years-old for young people with disabilities. It provides advice, and access to leisure, education and employment opportunities to help young people make a smooth transition to adulthood and working life.
Personal advisers
For young people with a Statement of Special Educational Need, a Connexions Personal Adviser will provide ongoing support throughout the transition process. They can make links with adult social and health care services and other agencies, and give advice on benefits and financial support. An Adviser is also able to support a young person as they move into further education or training. If your child is in education, Connexions will automatically become involved with them at the right time.
