Transition from Children's to Adults Services

Children's Services will support young people up to the age of 18 years-old. Many continue to require services and help beyond the age of 18. In order that the transition from Children to Adult Services happens smoothly, the process may begin several years before. This enables a young person, their family, carers and professionals to look at all of the different care, living, education and employment opportunities available, and adjust to the changes that will occur.  

Transition workers

There are several Adult Social Care Teams who may provide services. In order to help the process of transition from Children's Services, a Transition Worker works jointly with the Learning Difficulties and Disabilities Service (LDD), and Adult Service Teams. A Transition Worker is also a member of the team. Links are maintained with Children's Services, local schools and organisations to ensure that young people who may be in need of future advice and support are in touch with the team.

Start of the process

Children with a Statement of Special Educational Need will begin to discuss the transition process at their annual school review, from the time that they are 14 years-old. Adult Service professionals will be involved with these young people from the time that they are 16 years-old, as they make the transition over to Adult Services. Young people who do not have a Statement, will still receive support from their school and the Connexions Service during this time.

The Connexions Service and the Wokingham Borough Employment Service provides information about employment opportunities for young people.

Transition handbook

The transition handbook has been written to support families with young people with additional needs as they move on to adult life. It covers the key areas, including social care, education, employment, benefits, housing and support. All young people with a Statement of Special Educational Need will receive a copy of the Handbook before their annual school review when they are in school year 9.
