Eligibility criteria

The service supports children and young people, from birth to 18 years, who:

  • Have a permanent and substantial disability
  • Have a diagnosis of Autistic Spectrum Condition
  • Have complex health needs, including those awaiting transplant, cancer and palliative care
  • Have occupational therapy needs relating to their permanent and substantial disability
  • Have a Statement of Special Educational Needs (SEN) and receive more than 20 hours support a week in a mainstream school (due to their disability)
  • Attend Wokingham schools with specialist provision - Addington Special School, All Saints Primary or Lambs Lane Primary resourced for children with physical disabilities, Westcott Infant and Westende Junior resourced for children with a diagnosis of Autistic Spectrum Condition
  • Attend other Local Authority or non maintained/independent special schools (due to their disability - not behavioural emotional social difficulties)
  • Require a Statement of Special Educational Need where those needs are severe and/or complex and are considered so substantial that they cannot be met by the resources normally available to schools
  • Are 14 years and over and will meet adult Community Care Services criteria due to their disability
