Disabled Children's Team

The Team

The Disabled Children's Team - DCT - is a small, specialist team consisting of Social Workers, Occupational Therapists, a Community Learning Disability Nurse, specialist Carers Worker, and skilled support staff. Transition Workers from Adult Services also work closely with the Team. The Team is part of the Learning Difficulties and Disabilities Service - LDD.

It provides and co-ordinates a variety of services for disabled children and their families who live in the Wokingham Borough. It works together with a child or young person, their family, carers and other appropriate agencies.

Assessment and Services

When you initially contact the Team, it will need to find out information about your circumstances and the help your child and family require. This is called an assessment of need. See Eligibility and Assessments for more details on the process.

The Team will listen to your enquiry, offer specialist information and advice and suggest other organisations that might be able to help if it cannot assist you

Depending on the result of the assessment, the team may then arrange support or services for your child and your family. This could include: 

  • Equipment and adaptations

  • Short breaks both within and outside of the family home

  • Signposting to other services

  • Behavioural support

  • Carers support and information

  • Direct Payments

  • Specialist advice

  • Help and support from a social worker or occupational therapist

 

Eligibility

To be eligible for a service from the Disabled Children’s Team, the following criteria have to be met (this is currently under review):

  • The child or young person has permanent and substantial disability

  • The child or young person is under the age of 18 years and they and their family are resident in the Wokingham Borough

  • The child or young person needs more attention and support than a child of the same age and gender

 

As you are caring for a disabled child, you may also request a carer's assessment to be completed. As part of the carer’s assessment, the Team will look at your needs as well as your child's needs. More information is available in Carer Support.
