The Disabled Children's Team is a specialist team consisting of Social Workers, Occupational Therapists and skilled support staff.
It provides and co-ordinates a variety of services for disabled children aged from birth to 18 years-old and their families, who live in the Wokingham Borough. It works together with children, young people, families, carers and other agencies.
Eligibility
To be eligible for services, children must:

Have a permanent and substantial disability

Be under the age of 18 years, and they and their family are resident in the Wokingham Borough

Require more attention and support than another child of the same age and gender
If your child is not eligible, there are a number of local organisations and charities that can help. Information about these can be requested from the Team at any time, or by contacting the CAN - Children with Additional Needs - Network on 0118 974 3750.
Assessment
When you contact the Team, you will be put through to the Duty Officer who is a member of the professional staff. They will complete an initial enquiry form and ask you for some details about your situation. If you are looking for information and advice they may be able to give you this immediately.
If you are requesting a service, the Team will need to carry out an assessment of need. This means that they will look at your child's and family's needs, and will work out a plan with you to provide services or support that are best suited to meet these.
The Duty Officer is available from 9am to 5pm, Monday to Friday. If you request an assessment this should be completed in seven working days, although it may be longer or shorter depending on the situation.
