About the Transition Handbook
 
It covers the age range 14 - 25, when young people leave full time Education and transfer from Child to Adult Services in Social Care and Health Services.
 
This handbook has been produced in order to help guide a young person and their family through these changes. 
 
It contains information about services for disabled people in Wokingham Borough and explains the different stages of the Transition process.
