Disabled Children's Team

Who are we?
We are a small specialist team consisting of social workers and occupational therapists.
We provide and co-ordinate a variety of services for disabled children and their families who live in Wokingham Borough.
We work in partnership with the child/young person, the family, carers and appropriate agencies.
Assessment and services
Initially, we need to find out information about your circumstances and the help your child/young person and your family require. This is called an assessment of need.
Depending on the result of the assessment, we may then arrange support or services for your child/young person and your family.
These could include:
- Signposting to other services
- Specialist advice
- Equipment and adaptations
- Short breaks
- A variety of support services
- Help and support from a social worker or occupational therapist
Who is entitled to services?
To qualify for a service from the Disabled Children’s Team, the following criteria have to be met:
- The child/young person has permanent and substantial disability
- The child/young person is under the age of eighteen and both he/she and their family are resident in Wokingham Borough.
- The child/young person needs more attention and support than a child of the same age and gender
What you can expect from our services
- We will listen to your enquiry, offer specialist information and advice and suggest other organisations which might be able to help if we cannot assist you
- If you need more than advice, we will talk to you about your situation in detail - an assessment. We will do our best to assess your situation between 7 to 35 days, although, this can sometimes take longer - we will keep you informed if this is the case
- Following the assessment, we will explain what will happen next and confirm this is writing - this is called a care plan
- As you are caring for a disabled child, you may also ask for a carer's assessment to be completed. As part of the carer’s assessment, we will look at your needs as well as your child's needs.
